The Forward Performance Difference

All treatment sessions are by appointment only so we can be prepared to provide the best quality of care possible for our patients.
Initial evaluations as well as follow up visits are one hour. Dry needling package sessions are 30 minutes.We value your time, and we make it a point to be punctual and not wait around to be seen. If your appointment time is at 9 a.m., you’ll be seen by the doctor at 9 a.m. We only have a few chairs in our waiting area because the goal is for you not to be sitting in them for long.
Please wear comfortable clothing that you can move in and allow the doctors to have easy access to the treatment area. A good example would be something you wear to a gym class or go for a run. Our doctors are experts in evaluating movements and they will need to see how you move without having to struggle against restrictive clothing.
No! Arizona is a direct access state which means a doctor’s referral is not required to be seen for physical therapy or performance services. As Doctors of Physical Therapy, we have the knowledge base and tools required to treat injuries and conditions without prior intervention of a medical doctor/physician. If the clinician determines that the attention of a medical doctor is needed, we will make sure to refer you to your primary care physician or one of our specialists for follow up.
First, you and the doctor will confirm the details you shared during your phone consultation to gain a clear understanding of why you sought physical therapy. Second, the doctor will evaluate your movement patterns and prescribe a customized program that’s specific to solving your problem. We like to think outside the box to achieve your goals and do not practice cookie-cutter physical therapy. It is up to the doctors’ discretion if they choose to do hands-on treatment during the initial evaluation. It greatly depends on what they discover and what you feel comfortable with. All our doctors are dry needling certified, but that doesn’t mean you’re going to get needled during this appointment. It is up to the doctors’ clinical judgment as to what treatment will best address your deficits. Our doctors’ also pride themselves in their communication with their patients and maintaining it throughout your plan of care. If you think of a question after your session has concluded, no problem– feel free to give us a call or email your doctor directly and you’ll get a timely response.
We prefer to set up a phone consultation with you prior to your first visit to make sure Forward Performance Sports Physical Therapy is the best fit for your needs and can best focus on your issues. We would also like to get more insight and context of your injury or performance history so we can make the best use of your time during your first visit. At the end of your initial in-person session, your doctor will discuss with you scheduling and future visits with you.
We provide physical therapy and performance services which includes spinal and extremity manipulation, joint mobilization, mobilization with movement, dry needling, electro-dry needling, static and dynamic cupping, soft tissue mobilization, scrapping and much more! Our rate is our rate. You won’t be charged extra depending on the treatments during your visit. We use the tools that will make you better. Period.
We know that life happens sometimes and can cause your plans to change unexpectedly. We understand! If you let us know 24 hours ahead of time, then no cancellation fee will be applied. Since we only see one patient at a time this allows us enough time to schedule another patient and help them get the treatment that they need to get better. If you no-show or cancel for any reason less than 24 hours before your session, then 100% of your visit will be charged. These fees are non-refundable and cannot be applied to visits or packages.
Please contact us as soon as possible so we know you are ok and not to worry. If you arrive under the 15-minute grace period you can still be seen, but the doctor will not go over the timeslot you were originally scheduled for because other patients are likely waiting to be seen. Sessions cannot be pro-rated or minutes added onto another visit. If you arrive more than 15-minutes past your scheduled appointment time, you will need to reschedule your session. This will be treated as a no-show and will be charged the full visit fee. At FPSPT we highly value and respect the time of each and every patient.
We take a highly-individualized approach and dedicate 4-times more one-on-one time with a highly-trained and experienced physical therapist compared with traditional practices. This approach is a better fit outside the insurance provider model. We realize this model is new to many, but why is it especially great for performance-based rehabilitation? Here are a few scenarios that should help to explain why:
  1. Would you prefer to have the 1-on-1 attention of a doctor of physical therapy OR the majority of your treatment being with aids or assistants because your physical therapist is juggling 2-4 other patients at the same time?
  2. Would you prefer to have the undivided focus of the doctor when exercising to ensure proper technique OR have an aide give you an exercise and then walk away because they are helping 2-4 other patients simultaneously
  3. Would you rather have a full body functional movement analysis performed to diagnosis the cause of your issues OR ‘pick which part hurts the most’?
  4. Would you prefer to commute to and from the clinic 1-2 times per week OR 3 times per week?
  5. Would you rather be seen for 6-12 visits OR 20-30 visits?
The fact we don’t spend time negotiating with insurance companies allows our skilled doctors to be 1-on-1 with patients for a full hour to properly analyze and diagnose the root cause of your issues. Our patients usually only need to come in once or twice a week, for 6-12 visits to get back to yoga/pilates classes, working out, running, golfing, or any other activities they love.
Even though we do not submit insurance reimbursement claims in office, FPSPT can provide you with a ‘superbill’ to submit towards your claim at your time of service (with the exception of Medicare/Medicaid and some HMOs). Most of our patients have private insurance and with a superbill, are typically reimbursed between 50-80% of the cost of your visit, depending on the insurance company and if you have met their deductible. It is the patient’s sole responsibility to submit the claims and there is no guarantee that your insurance provider will reimburse you for the services, but we are happy to provide the documentation if you choose.

A superbill contains all the necessary information: business name and address, tax ID, national provider identification, license numbers, as well as the patient’s ICD-10 (diagnosis) and CPT (billing) codes for your claim to get reimbursed. You may submit bills to your insurance provider following each visit, one time per month, or at any other interval, typically for up to one year following your treatment session. It is the patient’s responsibility, not FPSPT, to submit all claims and to provide payment in full at the time services are rendered. Medicare, Medicaid and some HMOs, will NOT provide payment for services received “out of network”.

Superbills are not permitted for use with Medicare/ Medicaid, and some HMOs. If you are planning on submitting for reimbursement, please take the time to understand the terms and conditions of your eligibility and process through your insurance provider. Insurance companies reimburse patients according to their out-of-network benefits structure. Reimbursement benefits vary depending on the provider.

Your doctor will discuss visit prices with you during your intake call. We offer a variety of options, including packages and memberships, to best fit your needs. Every patient is unique, both in needs and goals, so recommendations vary as well.
YES! All visits and the majority of membership packages must be used by the same patient and within 1 year of date of purchase. All unused visits after 1 year are non-refundable.
